Minimum Requirements
Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Current Physical Therapy licensure in the state of Massachusetts. Current CPR certification. Current driver’s license, vehicle insurance, and access to a dependable vehicle for patient visits. Experience in home health settings, with emphasis on post-acute hospitalization and assisted living populations preferred. Ability to work autonomously and onboard quickly preferred. Minimum education: Bachelor’s Degree required.
Assignment Details
Facility type: Home Health. Shift name: Evenings. Shift details: Evenings . Weekly hour guarantee: 32 hours. On call and weekend requirement: Yes. Coverage area: Boston and surrounding areas; visits across agency service area required. Key responsibilities: evaluate functional status and systems for CMS documentation, develop PT plan of care, provide evidence-based treatment to relieve pain and restore/maintain function, document progress notes per policy, and communicate changes to nursing and physicians.
