Registered Nurse Patient Navigator

Carle Health seeks an RN Navigator to guide patients through the care continuum in our Healthcare & Medical Services department. This role coordinates care plans, educates patients and families, and facilitates communication among providers to improve outcomes and patient experience. The RN Navigator assesses clinical needs, removes barriers to care, and supports timely appointments, diagnostics, and treatments. Working closely with an interdisciplinary team, this nurse champions patient safety, quality standards, and evidence-based practice while contributing to program development, data tracking, and performance improvement initiatives.
