In-Home Traveler Nurse Practitioner or Physician Assistant

Signify Health is seeking a part-time In-Home Traveler Nurse Practitioner or Physician Assistant to complete comprehensive health evaluations for Medicare and other vulnerable populations in patient homes. You’ll perform head-to-to-toe assessments, review medical histories, reconcile medications, and identify care gaps using Signify’s technology platform. Collaborating with an interdisciplinary team, you’ll document findings in the EHR, support care coordination, and provide education that drives value-based care. This role offers flexible scheduling, autonomy in the field, and the opportunity to improve outcomes while traveling within your assigned region.
