In-Home Nurse Practitioner

Signify Health is seeking an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive clinical evaluations in patients’ homes. In this role, you’ll assess medical history, medications, functional status, and social determinants of health, identifying gaps in care and opportunities to improve outcomes. You will use Signify Health’s technology and analytics to document visits, inform value-based care plans, and collaborate with interdisciplinary teams.
