Signature Eugene Home Health is seeking a compassionate and dedicated Social Worker to join our interdisciplinary team. This role is ideal for a social worker passionate about  psychosocial support, and patient advocacy.

As a Home Health Social Worker, you will provide critical emotional support, care coordination, and community resource guidance to patients and families navigating life’s most meaningful transitions.

🔎 What You’ll Do (Key Responsibilities)

• Conduct comprehensive psychosocial assessments for patients and families
• Develop and implement care plans based on social and emotional needs
• Provide grief counseling, bereavement support, and crisis intervention
• Coordinate community resources, referrals, and support services
• Serve as a liaison between patients, families, and healthcare providers
• Collaborate with the interdisciplinary home health care team (RN, physician, aides)
• Participate in quality assurance and performance improvement initiatives

✅ What You Bring

• Licensed in Social Work (LCSW) from an accredited program
• 1+ year of healthcare experience (hospice or home health preferred)
• Strong communication, organization, and interpersonal skills
• Ability to work independently in a home health / field-based setting
• Current CPR certification
• Valid driver’s license and reliable transportation
