Home Health Speech Language Pathologist

CenterWell Home Health seeks a compassionate Home Health Speech Language Pathologist to deliver patient‑centered speech therapy in the home. In this role, you will assess, diagnose, and treat communication, cognitive‑linguistic, and swallowing disorders, develop and implement individualized care plans, and document progress in accordance with regulatory and payer requirements. You’ll collaborate closely with an interdisciplinary team, educate patients and caregivers, and support safe transitions of care.
