Join Touchstone Communities as an LVN Charge Nurse in our Memory Care Unit, providing compassionate, person-centered care to residents with dementia and Alzheimer’s. You will lead and coordinate nursing care on your shift, administer medications and treatments, monitor resident conditions, and respond promptly to changes in status. Collaborate with an interdisciplinary team to implement care plans, support families, and ensure regulatory compliance. Serve as a role model of servant leadership, guiding CNAs and fostering a culture of dignity, safety, and respect in a homelike, faith-based environment.
