Now Hiring: Full-Time Home Health Intake Coordinator
Clarksville, TN or Lebanon, TN
Schedule: Monday–Friday | 8:00 AM – 4:30 PM CST

As an Intake Coordinator, you’ll play a vital role in supporting patients and families by coordinating services that help individuals recover, manage chronic conditions, and remain safely in the comfort of their own homes.

Key Responsibilities
- Receive and process patient referrals by phone and in person
- Explain home health services and benefits to patients, families, and referral sources
- Assist in identifying alternative services when a patient is not eligible for care
- Coordinate and schedule initial nursing assessments and admissions
- Prepare and maintain admission packets and required documentation
- Support community outreach efforts and maintain strong referral relationships
- Provide administrative and clerical support to the clinical team
- Maintain accurate patient records and documentation in a timely manner
Qualifications
- Must be at least 18 years of age
- One (1) year of experience in home health, healthcare, or a related field preferred
- Strong organizational, communication, and customer service skills
- Ability to manage multiple priorities in a fast-paced environment
- Ability to complete documentation accurately and on time
- Valid driver’s license and proof of auto insurance required
