About the Role
Your work enables earlier clinical intervention and helps prevent avoidable hospitalizations, directly advancing Circadia's mission to bring high-quality, proactive care into patients' homes.
What You’ll Do
- Take and record patient vitals during home visits, ensuring accurate readings are captured and transmitted through the Circadia platform
- Oversee installation of Circadia devices at designated patient locations for our Home Health Program
- Facilitate patients' initial telehealth visits with our dedicated Home Health Nurse Practitioner
- Educate and empower patients to build comfort using the device and setting up telehealth visits
- Manage routine device maintenance, IT setup, configuration, and assembly of medical equipment
- Coordinate with internal teams throughout the customer lifecycle to ensure successful technology deployment
- Audit and maintain the asset management list
- Support the Procurement Team with supply chain planning for future installs and company growth
- Come into the office once a week, or as needed, to pick up supplies
Must-Have Qualifications
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification required, with demonstrated experience taking and recording vital signs (blood pressure, pulse, respiration, temperature, pulse oximetry)
- Skilled with power tools and equipment for mounting and installation
- Valid driver's license and reliable transportation (required for daily travel between patient homes)
- Comfortable working independently with minimal supervision
- Strong communication and customer service skills, especially with elderly or vulnerable populations
- Familiarity with HIPAA and patient privacy standards
- Willingness to pass a background check
- Mission-driven and passionate about helping patients get the proactive care they need
- Bilingual (Spanish) a plus, given the Orange County service area
