Physical Therapist Home Care

MedStar Health is seeking a Physical Therapist for home care services in Harford County. In this field-based role, you will assess, plan, and deliver evidence-based physical therapy to patients in their homes to restore function, mobility, and independence. Responsibilities include comprehensive evaluations, individualized treatment planning, gait and balance training, pain management, and patient/family education. You will document in EMR, collaborate with an interprofessional team, and support safe transitions of care while upholding MedStar’s patient-first, mission-driven standards.
