Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Home Health experience
• Experience using Matrix Care EMR

Assignment Details

• Schedule: 8:00 AM - 4:30 PM
• Shift Days: Monday - Friday
• Coverage area includes home visits within an approximate 10-15 minute radius
• Productivity expectation of at least 5 patients per day
