Outpatient Physical Therapist | Hospital-Based | Jacksonville FL | $5K Sign-On + PSLF

Position Highlights

• Freestanding hospital-based outpatient clinic, non-profit system (Jacksonville FL)
• PSLF eligible
• Mixed caseload: neuro (stroke, TBI, Parkinson's) and complex ortho
• $5,000 sign-on bonus plus relocation assistance
• 1:1 care, 45-min blocks, hard cap of 8 patients/day, no double-booking
• 4-to-6 week structured onboarding and mentorship
• M-F (4x10s or 5x8s), no weekend coverage, 5 weeks PTO

About the Role
Our ideal candidate has the heart of a servant leader and a passion for patient-centered care. We're not looking for someone after their next "job," but someone who wants to invest in their career and help patients understand what's happened to their bodies through their injury or condition. Patient education is the number one priority. You'll work alongside a large multidisciplinary team of 15+ PTs, OTs, and SLPs, with a $1,500 annual CEU allowance, a paid MedBridge subscription, and a retirement match.

Qualifications & Requirements

• Bachelor's, Master's, or Doctorate of Physical Therapy from an accredited program
• Active or pending Florida PT license
• Team player with a passion for patient-centered care
• Servant-leader mindset
