Home Health Physical Therapist

Optimal Care is seeking a Home Health Licensed Physical Therapist to deliver one-on-one rehabilitative services in patients’ homes. In this role, you will perform comprehensive evaluations, design individualized treatment plans, and provide evidence-based interventions to improve mobility, strength, balance, and functional independence. You will educate patients and caregivers, document care in EMR, and collaborate closely with an interdisciplinary team to ensure safe, goal-driven outcomes and reduce hospital readmissions.
