Key Responsibilities:

* Provide daily clinical oversight of patient care services. * Coordinate referrals and admissions with internal and external sources. * Evaluate patient needs and agency capabilities before admission. * Supervise care planning, partner assignments, and care delivery. * Ensure compliance with regulatory standards and agency policies. * Mentor and coach clinical staff to promote morale and retention. * Utilize MatrixCare and Strategic Healthcare Partner systems for workflow and reporting.

Required:

* Active, unrestricted RN license in the state of practice. * Minimum of two years RN experience (preferred) * At least one year of Home Health experience (preferred) * Current CPR certification.
