We are building a Program of All-Inclusive Care for the Elderly (PACE) to bring life-changing, culturally competent care to low-income seniors—supporting them as they age safely and with dignity, right in their communities. Based on the PACE model, SpringLight Health will offer coordinated medical care, transportation, meals, social activities, medication management, and caregiver support—all tailored to each individual’s needs.

The Activity Coordinator is responsible for providing a comprehensive therapeutic recreation program for PACE Participants. This includes assessment, care planning, and the coordination of therapeutic and recreational activities for individuals and groups.
The PACE Activity Coordinator is a member of the PACE Interdisciplinary Team (IDT) and is responsible for conducting recreational therapy assessments for new PACE participants and reassessments for existing PACE participants as part of the PACE IDT care planning process.
Essential Job Functions
Minimum Requirements
- Fluency in Chinese (Cantonese or Mandarin) is required. (Fluency in other languages is an asset.)
- At least two years of experience in a social recreational or educational program within the past five years, one year of which was full-time in an activities program in a health care, mental health, or handicapped program setting.
- Bachelor’s degree in Recreational/Art/Music/Dance Therapy, Psychology, Social Work or a related human services field from an accredited university or college.
- Ability to make home visits as assigned.
- Knowledge of social activities, including programming tailored to the older adult population.
Preferred Qualifications
- At least 1 year of experience working with the frail or elderly population.
- Certification by the National Council for Therapeutic Recreation Certification or the California Recreation Therapist (RTC).
- Strong understanding of the psychosocial dynamics in aging.
Certifications, Licenses, Registrations
- Current BLS and First Aid required.
