Community Care Physicians is seeking an Outpatient Vascular Nurse Practitioner or Physician Assistant to provide patient-centered care in New York’s Capital Region. Assess patients with vascular conditions, review diagnostic findings, develop and manage treatment plans, provide education, and coordinate care with physicians and the clinical team. The ideal candidate holds an active New York State NP or PA license, is board certified, and has outpatient vascular experience.
