Job Description:
Physical Therapist Assistant
Make a meaningful difference by helping patients regain strength, mobility, and independence in the place they feel most comfortable—their own homes.
Elara Caring is seeking a compassionate and motivated Home Health Physical Therapist Assistant (PTA) to provide personalized, one-on-one therapy under the direction of a licensed Physical Therapist. You’ll enjoy the independence of working in the field while receiving support from an experienced, collaborative clinical team.
Full-Time | Monday–Friday | Newton-Waltham Territory

What You’ll Do
Provide skilled physical therapy services according to the treatment plan established by the supervising Physical Therapist.
Help patients improve mobility, strength, balance, and overall function.
Review the Plan of Care before each visit and follow physician orders and established clinical guidelines.
Monitor patients’ progress and promptly communicate changes in condition to the Physical Therapist.
Teach patients and caregivers how to safely complete prescribed home exercise programs.
Provide instruction on the safe use of walkers, canes, wheelchairs, prosthetics, orthotics, and other assistive devices.
Accurately document visits, patient progress, and response to treatment.
Follow all state practice requirements and organizational policies.
Collaborate with the Physical Therapist and interdisciplinary care team to support positive patient outcomes.
What You’ll Need
Graduate of an accredited Physical Therapist Assistant program.
Current, unrestricted PTA license in the state of practice.
At least six months of PTA experience preferred.
Home health experience preferred.
Reliable transportation, a valid driver’s license, and current automobile insurance.
Ability and willingness to travel throughout the assigned territory.
Sit, stand, bend, lift and move up to 50 pounds, including positioning or transferring patients and moving equipment
