CenterWell Home Health seeks a PRN Home Health Physical Therapist to deliver one-on-one, in-home therapy for seniors and adults with chronic conditions. You will assess functional status, create and adjust individualized care plans, and provide evidence-based interventions to improve mobility, safety, and independence. Collaborating with an interdisciplinary team, you’ll educate patients and caregivers, document progress, and ensure high-quality, patient-centered care aligned with clinical and regulatory standards in a flexible PRN role.
