Physical Therapist

Heritage Health Care Center, part of HHP Lifeshare, is seeking a licensed Physical Therapist to join our Healthcare & Medical Services team. In this role, you will evaluate patients, develop individualized treatment plans, and deliver evidence-based therapies to improve mobility, function, and overall quality of life. You’ll collaborate closely with an interdisciplinary care team, educate patients and families, and document progress accurately.
