Under the supervision of a licensed clinical professional, the Care Management Analyst (CMA) plays a vital role in the Comprehensive Kidney Care Contracting (CKCC) Program, supporting patients across the continuum of kidney care. This highly collaborative position combines patient engagement, care coordination, and population health management to help patients navigate complex healthcare needs, improve access to care, and achieve better health outcomes.
The CMA serves as a trusted resource for patients, caregivers, providers, and interdisciplinary care team members. Through proactive outreach and coordination, the CMA helps ensure that patients receive timely care, remain engaged in treatment plans, and have access to the resources needed to successfully manage their kidney health.
This is an excellent opportunity for an LPN who enjoys building relationships with patients, coordinating care across multiple settings, and making a direct impact on quality outcomes and patient experience.
Work Location:
This position is primarily remote and focuses on telephonic outreach and care coordination with CKCC program participants. Occasional travel to provider offices, healthcare facilities, team meetings, or other sites of care may be required based on program or business needs.
Key LPN role in CKCC program includes:
Engage and educate patients about the CKCC program, ensuring understanding of program benefits and completing enrollment and consent activities.
Build meaningful relationships with patients and caregivers through ongoing outreach and support.
Coordinate care across providers, dialysis centers, hospitals, and community resources.
Assist patients in overcoming barriers to care by facilitating:
Appointment scheduling and follow-up visits
Medication access and prescription refill coordination
Transportation arrangements
Referrals to appropriate healthcare and community services
Support smooth transitions of care following hospitalizations through post-discharge outreach and scheduling of timely follow-up appointments.
Facilitate completion of Patient Activation Measure (PAM) assessments to support patient engagement and self-management.
Coordinate referrals and transitions for patients initiating outpatient dialysis services.
Gather and organize clinical and non-clinical information from patients, caregivers, providers, electronic health records, and other systems to support care management activities.
Collaborate closely with multidisciplinary care teams to identify opportunities for intervention and improve patient outcomes.
Serve as a resource for patients, physicians, caregivers, and internal stakeholders regarding CKCC program services and processes.
Requirements:
Associates degree required. Bachelor’s Degree preferred.
LPN Licensure Required.
Current active license/registration in New York State.
Medical Management experience in a managed health care setting preferred.
Appropriate training and experience in case management preferred.
