CenterWell Home Health seeks a compassionate Occupational Therapist to provide patient-centered, in-home rehabilitative care. In this role, you will assess functional abilities, develop individualized treatment plans, and deliver evidence-based interventions to improve safety, independence, and quality of life. Collaborate closely with an interdisciplinary team, educate patients and caregivers, and ensure accurate, timely documentation.
