Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations in patients’ homes. You’ll complete detailed medical, functional, and social assessments, review medications, and identify gaps in care for primarily Medicare and other vulnerable populations. Using Signify Health’s tech-enabled, value-based care platform, you’ll document in EHRs, develop evidence-based recommendations, and collaborate with primary care and care coordination teams.
