Registered Nurse

Home Health of Western Colorado in Grand Junction is seeking a Registered Nurse to provide skilled, compassionate in-home care across Western Colorado. The RN will complete comprehensive patient assessments, develop and update care plans, administer medications and treatments, and provide wound, post-surgical, and chronic disease management. This role collaborates closely with an interdisciplinary team, educates patients and families, and documents care accurately. The nurse will triage patient needs, coordinate services, support safe transitions of care, and help individuals maintain independence and quality of life at home.
