In-Home Traveler Nurse Practitioner or Physician Assistant

Signify Health is seeking a part-time In-Home Traveler Nurse Practitioner or Physician Assistant to perform comprehensive, value-based health evaluations in members’ homes. You’ll travel within assigned regions to complete head-to-toe assessments, review medical histories, reconcile medications, and identify care gaps for primarily older and vulnerable populations. Using Signify’s technology and analytics-driven tools, you will document findings in the EHR, support risk adjustment accuracy, educate patients and caregivers, and communicate key insights to health plans and primary care teams to improve outcomes and reduce avoidable costs.
