Lead Case Manager RN

TriStar Health seeks a Lead Case Manager RN to coordinate complex patient care across our Nashville network. In this role, you’ll oversee clinical case management, guide safe transitions of care, and support utilization review for diverse inpatient populations. You’ll collaborate with physicians and interdisciplinary teams to optimize outcomes, reduce readmissions, and connect patients to needed resources. As a leader, you will mentor case managers, drive process improvements, and model evidence-based, compassionate practice within our culture of safety and continuous learning.
