Home Health Licensed Physical Therapist

Optimal Care is seeking a compassionate Home Health Licensed Physical Therapist to provide one-on-one rehab services in patients’ homes. In this role, you will complete comprehensive evaluations, design individualized treatment plans, and deliver evidence-based interventions to restore mobility, strength, and independence. You will educate patients and caregivers, ensure accurate EMR documentation, and collaborate closely with an interdisciplinary team.
