About the Role:

We are seeking a dedicated and compassionate Evening LPN to join our team. As an Evening LPN, you will play a vital role in delivering high-quality care to our guests, while maintaining a comfortable and safe environment. You will work closely with our interdisciplinary team to assess, plan, and implement effective care strategies, promoting our guests' overall well-being.

Responsibilities:

  • Provide direct patient care to our guests, including vital sign monitoring, medication administration, and wound care

  • Develop and implement individualized care plans, working closely with our interdisciplinary team

  • Maintain accurate and detailed records of patient care and progress

  • Communicate effectively with guests, families, and healthcare professionals

  • Participate in quality improvement initiatives to ensure the highest level of care

  • Collaborate with our rehabilitation team to support our guests' recovery and independence

Requirements:

  • Current LPN/LVN license in the state of Ohio

  • Minimum 1 year of experience in a skilled nursing setting

  • Strong communication and interpersonal skills

  • Ability to work effectively in a fast-paced environment

  • Strong attention to detail and organizational skills
