About the Role:
We are seeking a dedicated and compassionate Evening LPN to join our team. As an Evening LPN, you will play a vital role in delivering high-quality care to our guests, while maintaining a comfortable and safe environment. You will work closely with our interdisciplinary team to assess, plan, and implement effective care strategies, promoting our guests' overall well-being.
Responsibilities:
Provide direct patient care to our guests, including vital sign monitoring, medication administration, and wound care
Develop and implement individualized care plans, working closely with our interdisciplinary team
Maintain accurate and detailed records of patient care and progress
Communicate effectively with guests, families, and healthcare professionals
Participate in quality improvement initiatives to ensure the highest level of care
Collaborate with our rehabilitation team to support our guests' recovery and independence
Requirements:
Current LPN/LVN license in the state of Ohio
Minimum 1 year of experience in a skilled nursing setting
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment
Strong attention to detail and organizational skills
