Physical Therapist

Centerpoint Health, a mission-driven community health center in Middletown, Ohio, seeks a Physical Therapist to provide evidence-based rehabilitative care to underserved populations. In this role, you will evaluate and treat patients with musculoskeletal, neurologic, and functional limitations, creating individualized care plans to improve mobility, reduce pain, and restore function. You’ll collaborate closely with primary care, behavioral health, and other disciplines, document care in our EHR, and educate patients and families on home programs.
