Signify Health is seeking an In-Home Physician to deliver comprehensive, value-based care to vulnerable patients where they live. In this role, you’ll perform head-to-toe clinical evaluations, review medications, manage chronic conditions, and develop collaborative care plans in partnership with health plans and providers. You’ll document visits using modern digital tools, leverage data and analytics to close care gaps, and coordinate with interdisciplinary teams.
