Sagility is seeking a Senior Care Specialist to support health plan members and patients through high-quality, technology-enabled care coordination. In this role, you will provide telephonic and digital support, answer questions on benefits and services, and help close care gaps through proactive outreach. You’ll collaborate with clinical and care management teams, document interactions in care management and CRM systems, and use digital tools and analytics to improve member experiences and outcomes. Working in Sagility’s people-first, inclusive culture, you’ll contribute to process improvements, meet performance and quality goals, and help deliver better healthcare experiences for members every day.
