FOX Rehabilitation seeks a Physical Therapist to provide evidence-based geriatric rehab in patients’ homes and senior living communities. Evaluate, develop, and implement individualized treatment plans focused on functional outcomes, fall prevention, and safety. Collaborate with an interdisciplinary team, educate patients and caregivers, and document care using EMR.
