Full job description

CenterWell Home Health seeks a Home Health Occupational Therapist Assistant to deliver compassionate, high-quality therapy in patients’ homes. In this role, you’ll follow OT care plans to help adults and seniors regain independence, safety, and confidence in daily activities. Responsibilities include implementing therapeutic exercises, training in ADLs, documenting progress, and educating patients and caregivers. You’ll work closely with an interdisciplinary team committed to patient safety, integrity, and continuous improvement, in a supportive environment that promotes collaboration, learning, and professional growth.
