Home Health Occupational Therapist
Full Time/ Plymouth Territory

What You’ll Do
Evaluate patients’ functional abilities and rehabilitation needs
Develop personalized plans of care focused on safety and independence
Help patients improve daily activities such as dressing, bathing, cooking, and mobility
Teach patients and caregivers how to safely use adaptive equipment
Recommend equipment and home modifications that make daily life easier
Monitor progress and adjust treatment plans as patient needs change
Communicate important updates to physicians and other members of the care team
Coordinate safe and effective discharge planning
Provide clinical direction to Certified Occupational Therapy Assistants and other care team members
What You’ll Need
Graduate of an accredited Occupational Therapy program
Current, unrestricted Occupational Therapist license in the state of practice
At least one year of clinical care experience
Ability to travel throughout the assigned territory
Dependable vehicle, valid driver’s license, and current automobile insurance
Ability to travel up to 50%
Flexibility to work varying or on-call hours when applicable
Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
