Full job description
Minimum Requirements Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active West Virginia state physical therapy license required to start the assignment. BLS certification from AHA or ARC may be required. Home health experience required. Experience with HCHB (Homecare Homebase) required. Minimum education requirement: Bachelor‘s Degree. Dress code scrub color: navy blue (client-specific requirement).
Preferred Qualifications Additional home health clinical experience preferred. Prior use of HCHB preferred beyond basic competency.
Assignment Details Shift hours: . Schedule: 8 hours per day, 5 days per week. Mon-Fri 8:00 am–4:30 pm noted by client. Coverage areas include Putnam, Cabell, and Wayne counties. No on-call or weekend requirement. Mileage reimbursed between visits from first patient to last patient.

Impacting Patient Care Provide skilled home health physical therapy to patients across the Huntington service area. You will document care using HCHB and coordinate with interdisciplinary teams to support safe discharge planning and functional improvement. This role supports continuity of care in community settings and contributes to patient independence and quality of life.
