At Home Physical Therapist

JAG Physical Therapy is seeking an At Home Physical Therapist to deliver high-quality rehabilitative care to patients in their homes across New Jersey. In this role, you will evaluate patient conditions, develop individualized treatment plans, and implement evidence-based interventions to improve mobility, function, and quality of life. You will educate patients and caregivers on exercises, safety, and equipment use, while maintaining accurate clinical documentation.
