In-Home Health Assessment Physician Assistant

Signify Health seeks a Physician Assistant to perform comprehensive in-home health assessments for vulnerable patient populations. You’ll obtain histories, perform physical exams, review medications, and document findings to support accurate risk adjustment and care planning. Partnering with interdisciplinary teams, you’ll identify gaps in care, educate patients, and coordinate follow-up with primary and specialty providers.
