Registered Nurse

Community Care Physicians, a physician-owned multispecialty medical group in Latham, NY, is seeking a Registered Nurse to deliver patient-centered care across our Capital Region practices. The RN will provide comprehensive outpatient nursing care, including assessments, medication administration, care coordination, and patient education in collaboration with physicians and support teams. You’ll document in our EMR, support quality and safety initiatives, and help patients manage acute and chronic conditions.
