Locums PA - Internal Medicine Position in IN

Covers 3 locations per day -- approx. 6 locations per week
Total patient panel of 340 patients across 8 locations
Multiple facility locations -- travel required throughout the week
Patient population: ages 55 and older
Chronic disease management required -- HTN, DM, CHF, COPD, CKD, CAD
GI management required -- GERD, constipation, malnutrition, incontinence
Geriatric post-acute care experience required
Leave coverage -- 6-week assignment
