Medical Social Worker

Satellite Healthcare, Inc., a mission-driven not-for-profit kidney care provider, seeks a Medical Social Worker to support patients receiving dialysis. In this role, you’ll assess psychosocial needs, provide counseling, and coordinate community resources to help patients and families cope with chronic kidney disease. You’ll collaborate closely with nephrologists, nurses, and dietitians, support treatment adherence and transitions of care, and advocate for patients.
