Job Description:
PRN Home Health Speech-Language Pathologist (SLP)
Help Patients Find Their Voice, Confidence, and Independence at Home
Make a meaningful difference while enjoying the flexibility of a PRN schedule. As a Home Health Speech-Language Pathologist, you’ll provide personalized, one-on-one care to patients in the setting where they feel most comfortable—their own homes. You’ll assess and treat communication, cognitive, and swallowing disorders while educating patients and caregivers on techniques that support safety, independence, and quality of life.
What You’ll Do
Evaluate patients’ speech, language, cognitive, communication, and swallowing needs.
Develop individualized treatment plans based on physician orders and assessment findings.
Provide skilled therapy services in patients’ homes.
Teach patients and caregivers exercises, techniques, and strategies to support continued progress.
Recommend and provide instruction on assistive devices when appropriate.
Monitor patient progress and update treatment plans as needs change.
Communicate changes in condition, progress, and recommendations to physicians and the interdisciplinary care team.
Coordinate services with nurses, therapists, caregivers, and other healthcare professionals.
Complete accurate and timely clinical documentation.
Maintain compliance with professional standards, regulatory requirements, and company policies.
What You’ll Need
Master’s degree in Speech-Language Pathology from an accredited program.
Current, unrestricted Speech-Language Pathologist license in the state of practice.
At least one year of SLP experience in a clinical care setting.
Valid driver’s license and an acceptable driving record, or reliable transportation to travel between patient homes and assigned locations.
Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
