License Clinical Social Worker

HarmonyCares is seeking a Licensed Clinical Social Worker to deliver compassionate, home-based care to medically complex and home-limited patients. In this role, you will conduct comprehensive psychosocial assessments, develop person-centered care plans, and provide counseling, crisis intervention, and ongoing support to patients and caregivers. You’ll coordinate community resources and benefits, address social determinants of health, and collaborate closely with physicians, nurses, and other interdisciplinary team members. This position blends in-home visits with telehealth and care management to improve outcomes, reduce avoidable hospitalizations, and enhance quality of life.
