Home Health Social Worker

CenterWell Home Health, part of Humana Inc., is seeking a Home Health Social Worker PRN to support seniors and adults with chronic conditions in their homes. In this PRN role, you’ll conduct psychosocial assessments, develop care plans, and connect patients and families with community resources and support services. You’ll collaborate closely with nurses, therapists, and physicians to address emotional, social, and environmental barriers to care. Working in a compassionate, patient-first culture, you’ll help patients manage illness, navigate benefits, and maintain independence at home while enjoying flexible, as-needed scheduling and opportunities for professional growth.
