As a Case Manager RN at TriStar Health, you will coordinate safe, efficient care transitions for patients across our Middle Tennessee and Southern Kentucky network. You’ll perform clinical assessments, create individualized care plans, and partner with physicians, nurses, and social services to support optimal outcomes, appropriate utilization, and timely discharge.
