Care Extender / Peer Support Specialist - Foster Care

Henderson County

Location: North Carolina

Must reside in Henderson County.

This position is primarily virtual/working from your home office, with responsibilities that include community-based travel and in-home member visits to support member needs.

Hybrid 1: This role requires associates to be in-office 1 - 2 days per week, fostering collaboration and connectivity, while providing flexibility to support productivity and work-life balance. This approach combines structured office engagement with the autonomy of virtual work, promoting a dynamic and adaptable workplace.

The Care Extender / Peer Support Specialist - Foster Care is responsible for engaging with community partners and builds strategy with local stakeholders; providing education, strengthening relationships, and helping resolve case or broader systems issues. Supports care coordination teams and connect members and caregivers with those who can provide insight and expertise to assist in overcoming challenges.

How you will make an impact:

  • Identify opportunities for engagement of members and their caregivers and families to share with care management team.

  • Partners with peer support services in the community to create referral pathways and links members to appropriate community peer support services.

  • Support care manager as they advocate for foster parents and other caregivers, addressing their needs or challenges with various agencies and systems. Develop strategy for peer support programs.

Minimum Qualifications:

Requires a HS diploma or equivalent and a minimum 2 years of related experience; or any combination of education and experience, which would provide an equivalent background.
