In-Home Primary Care Nurse Practitioner or Physician Assistant

Signify Health is seeking a part-time In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for adult and senior members in their homes. You will complete detailed histories and physicals, assess chronic conditions, reconcile medications, and identify care gaps to support value-based care. Using Signify’s technology and analytics, you’ll document findings in the EHR and collaborate with care teams to inform individualized care plans.
