Care Manager Registered Nurse

Mission Health in Asheville, NC is seeking a Care Manager Registered Nurse to coordinate patient care across the continuum. The RN Care Manager assesses clinical and psychosocial needs, develops individualized care plans, facilitates transitions of care, and collaborates with physicians, therapists, social workers, and community resources. This role supports patient-centered goals, reduces readmissions, ensures appropriate utilization, and educates patients and families.
