Job Title: Licensed Practical Nurse Unit Manager

Role Overview:

The LPN Unit Manager is a critical leadership role responsible for overseeing the daily clinical operations of the rehabilitation unit. You will manage nursing staff, coordinate complex post-acute care, and serve as the primary liaison between residents, families, and the interdisciplinary therapy team. This role ensures high-quality clinical outcomes and maintains compliance with all state and federal regulatory standards.

Job Details:

  • Full-Time (Monday - Friday)

  • Day Shift

  • Salary Range: $36-$43/HR

Required Qualifications:

  • Licensure: Active and valid state Licensed Practical Nurse (LPN) license in good standing.

  • Certification: Current Basic Life Support (BLS/CPR) certification.

  • Experience: Minimum of 2 years of nursing experience in a Long-Term Care (LTC) or Skilled Nursing Facility (SNF) setting. Minimum of 1 year of experience in a supervisory or leadership capacity (preferred).

  • Skills: Strong clinical assessment skills, proficiency with Electronic Health Records (EHR), and the ability to lead a team in a high-stress environment.

Physical Requirements:

  • This position is active and requires standing, walking, bending, and the ability to lift or move patients weighing over 50 pounds.

Key Responsibilities:

  • Clinical Oversight & Care Planning: Oversee day-to-day patient care on the rehab floor, ensuring treatments and medications are administered according to physician orders. Collaborate with the Interdisciplinary Team (IDT) to assist in developing and adjusting individualized care plans.

  • Staff Supervision & Leadership: Supervise, mentor, and evaluate the performance of LPNs and CNAs on the unit. Manage staffing schedules, delegate daily assignments, and address any immediate staffing shortages.

  • Quality Assurance & Compliance: Ensure the unit meets all CMS and state regulatory standards. Perform clinical audits on medication administration, wound care, and infection control programs.

  • Communication & Liaison: Act as the main point of contact for residents and family members, addressing concerns or complaints with compassion and diplomacy. Effectively communicate with physicians and nurse practitioners regarding changes in resident status.
