Monday - Friday, 8am - 5pm, onsite 40 hours/week

Essential Functions:
- Assists with the scheduling of various inpatient and outpatient services, including tests, procedures, and follow-up appointments.
- Communicates with patients and families regarding admissions and appointments and connects them with appropriate resources.
- Collaborates with multidisciplinary teams to ensure cohesive care planning and effective communication throughout the discharge process.
- Obtains required information from patients, caregivers, healthcare providers, and other external teams.
- Assists with completing required documentation such as FMLA forms and obtaining payer authorizations for medications, equipment, and services related to post-discharge care.
- Monitors related metrics and assists with data gathering to support process and patient care improvements.
- Ensures compliance with regulatory and organizational privacy and safety standards.
Education Requirement:
Associate's Degree, or equivalent education and experience, required.
Skills:
- Knowledge of current medical terminology.
- Knowledge of payer negotiations and authorization requirements.
- Excellent communication and interpersonal skills.
- Able to work in a fast-paced clinical environment and manage multiple tasks effectively.
- Demonstrated effectiveness in working with multi-disciplinary teams.
Experience:
- Experience in an inpatient or outpatient clinical setting, preferred.
- Experience with electronic medical records, preferred.
