As an In-Home Nurse Practitioner or Physician Assistant at Signify Health, you will deliver comprehensive in-home evaluations and manage acute and chronic conditions for vulnerable populations. Using modern technology and data-driven insights, you’ll document visits in EHRs, create patient-centered care plans, and collaborate with interdisciplinary teams to advance value-based care.
