  • Covers 3 locations per day -- approx. 6 locations per week
  • Total patient panel of 340 patients across 8 locations
  • Multi-site facility coverage -- 8 locations throughout the week
  • Patient population: ages 55 and older
  • Chronic disease management required -- HTN, DM, CHF, COPD, CKD
  • GI management required -- GERD, constipation, malnutrition, incontinence
  • Geriatric post-acute care experience required
  • Leave coverage -- 6-week assignment
